Anyone traveling into Connecticut from a state with a positive case rate higher than 10 per 100,000 residents, or higher than a 10% test positivity rate over a 7-day rolling average, or from a country for which the Centers for Disease Control and Prevention (CDC) has issued a Level 3 Travel Health Notice, are directed to:
1. Self-quarantine for a 14-day period from the time of last contact within the identified state
2. Provide proof of negative COVID-19 test results within three days of arrival to avoid quarantining
3. Get tested in Connecticut and quarantine until receipt of negative test results

Residents of New York, New Jersey and Connecticut are exempt from this advisory but are advised to avoid non-essential travel.
Travelers will now be required to complete a form upon entry into Connecticut stating their name, date of birth, state of origin, estimated length of stay and location while in Connecticut, as well as contact information. The form is required to be submitted to DPH and can be completed online by visiting ct.gov/travelform.
